1. Complete the form here and email it to Muhammad Farid at mfa@nus.edu.sg
2. Thereafter, please come to BME office to collect the visitor access card
3. There is a $20 cash deposit for the visitor access card.
4. This amount will be refunded once the visitor access card is returned.
